Is this Velcrow or a way to tighten it? Or do you just slip it on?

Just slip it on. Either way it works.

Does this make plastic sound noise? Can it be worn to the gym?

It don't make any noise at all and it's pretty stretchy actually, it is probably one of the most comfortable ones to wear to the gym since it's not stiff at all.

My waist is 27.7 inches, should i get small or medium size?

Probably a medium because I wear a xl in tops im big in the mid section I ordered a large or xl and it barely fits and it doesn’t have zippers so be mindful of that. You have to put it on over your head
