can the toilet roll holder bar be reversed so that the open end is to the left when viewed from the front. (reverse of what the picture shows)

The holder does not come apart. The only way to reverse it would be mount to upside down, if you did that the mounting screw would be on top where it would be seen

Does it rattle much when unrolling paper? The Miseno version takes too much.

Doesn’t rattle at all. You set the holder at the position you want it from the wall and it stays in place unless you reset it to another distance. This is absolutely the best toilet paper holder I’ve ever had.

The mounting instructions show the screws need to align horizontally. Can it be installed with screws lining up vertically?

I believe that they need to be horizontal due to the configuration of the internal piece that is mounted to the wall. You will not be able to see the screws once it is mounted so this shouldn't be an issue.

Good quality but not the metal flap or cover is a bit small

The product is well made like all Grohe products. I would have given it a five star if the metal flap was 1.5 inches to 2 inches longer. The flap is a bit on the short side and does not hold the toilet paper firmly in place all the time.
