Is this for a 6 years old?

Yes Yes !! My granddaughters are 2 and 4 and they love it but can't always maneuver the blood pressure cuff, etc. A six year old would be the perfect age. The doctor's jacket is made very nice and washed and dried perfectly. This is a very good choice.

This jacket will fit a 2 year old. I would say would fit 2-4 years old.

Cant toy batteries be replaced?

I can only speak to the little flashlight right now, as it's the only one I've tried to replace the batteries on. It takes two LR1130 batteries, and can be accessed pretty easily if you have a sharp, flat, metal tool to pop open the compartment that houses them. Honestly a fork might work.

What is the quality of the doctor toys medical kit?

I was impressed with the quality given the price. A couple of the lights have stopped working but that hasn't kept the kids from playing with them.
