could i put an air stone inside and have the bubbles come out of it?

That's exactly what I did. There's a large enough hole in the back of the head to fit an air stone through. The only issue I had was that air would gather at the front of the nose and lift the whole head to let the air out. Solved that problem by burying the nose end further into the gravel and moving the air stone as far to the back of the head as I could.

Is it open in the back for the fish to swim in?

yes the back is open and its a good size. not to big but not to small

It’s medium the size of a large grapefruit

Is this saltwater aquarium safe?

I would not put this in any aquarium. The inside of the skull is all rough and pokey, if fish get In there they will RIP their fins.
