Can’t choose united states on set up, it is not an option. i chose canada, then i went in and updated it, but still no way to change it to us. help?

You were scammed by the seller.  Multiple sellers are selling on this listing; some are legit, while others (like East Hub Ltd) are scammers fraudulently selling the cheaper international unit (Loco5AC) instead of the FCC registered unit (Loco5AC-US) clearly advertised on the listing and legally required for use in the US.  You will have to return the defective equipment and buy from a more reputable seller in the future to solve this problem.  I would strongly recommend finding out who the seller was in your case (it will say on your Amazon order invoice) and leaving negative seller feedback on their profile saying what they did.  This is the only way to protect future buyers.

No this is just the Radio, you will need a passive PoE supply to support the radio.

Are these authentic for use in US? I see comments they are not.

Not sure what you mean by "authentic" but I use Ubiquiti equipment for many applications including the Ubiquiti NanoStation 5AC Loco, 5 GHz airMAX for Ethernet, Security, and CCTV applications all over DC, VA, MD, and PA area. It works great and is very dependable.

Does this work with a standard 802.3af PoE switch or does it require a proprietary power supply?

This actually only works with a 24v passive PoE supply. 802.af is 48v
The non-loco version which is twice the price is one of the few Ubiquiti PtP radios that supports both 24 and 48v power. 
However, ubiquiti makes an adapter for $20 you can use to connect the Loco (or any 24v passive device) to 802.3af switches.
