The nozzle which is supposed to spray the product has no orifice. I don't dare perforating it because I cannot control the orifice diameter.

Twist the end of the nozzle to the right. It is like a spray bottle it twists open and twist back to close.

What are the contents besides olive oil?

As far as I can tell the only ingredient is natural olive oil it is the delivery method that makes it an effective ear cleaner

When pressing the activator nothing happens, what to do?

I like the spray compared to drop type. Maybe yours does not work properly. Contact the vendor.
