Has anyone used the Amazon "assembly service" where someone installs this for you? If so, did it work well for you?

I never got a call from a service company. Eventually I got an email saying that my installation service was cancelled with a reason of "scope of work has changed". After some digging I found that Amazon had a hard time finding a company that does installation work answer their call. Amazon said that they tried 3 different companies. I have a hard time believing that there are only 3 companies that they can use in a major metropolitan area. The service was on sale for $102, which is much much cheaper than contracting a company myself. Not pleased.

Everywhere i read that the silverback 54" pole has to be cemented into the ground, but the 54" on amazon says it comes with the anchor kit. does it?

Yes it comes with everything you need to anchor the base in concrete, rebar, j bolts, anchor plate and nuts. You provide the hole and concrete.

Does the Silverback SB60 sold on Amazon come with an anchor bolt? Walmart says it has one so it can be moved if you move to another house

Mine came with 4 J bolts that you embed in the concrete footing. When the footing is cured, you can use the included nuts to attach the base of the post to the ends of the bolts that are sticking out of the footing. The vertical bolts are arranged in a square pattern to match up with the notches on the horizontal base of the post.The backboard and post are heavy when assembled, so plan on needing a few guys to move it. You'd need to buy another set of four bolts for the new house if you were to move it and have them spaced properly to hold down the base of the post.
