is this aspirator good for adult ears also?

I don’t care for this after using it. There are better ways. It is only ok for nose, but the two piece design, while easy to clean, comes apart too easily when you squeeze to suction, so it doesn’t get enough suction to pull out stuff. Ear wax is is thicker so this did little. I don't think it would be good for an adult’s ear at all.

Where are the directions? Does it require a liquid? And what kind?

It doesn’t take liquid. You insert it into the nostril and squeeze the bulb. I found it didn’t work - not adequate suction. I got the electric powered kind and it works great.
