Do these legens have a strap that goes under the shoe to hold them down

No strap under the shoe; the gaiter and front flap stay in place without the need for a strap.

How useful are they really? Can a snake still bite throught your shoe?

They provide protection, from just below the knee to your ankle and over the the tongue area of the shoe... If your shoes are not snake resistant you are certainly vulnerable there...

Has anyone use them for trail running?

Not that I know of; probably too bulky for running. And we advise that snake gaiters be worn with boots or heavy shoes that fangs could not penetrate -- not tennis or athletic shoes.

Have any petite women used these and can you confirm they do fit us? Unisex gear rarely does.

We have lots of women customers. Please measure the circumference of your calf while wearing the same type of pants you would normally wear outdoors. Razer Snake Gaiters fit calves 13-20 inches. The gaiters are about 12 inches tall measured from knee to ankle. If your measurements fall within these ranges, then they should fit you. If not, they are returnable if unworn.
