Why is there 4 holes but only 3 screws?

You are missing 1 screw, mine came in a pk of 4

Can the toilet paper easily slide off the pipe when unrolling toilet paper?

It doesn’t because you install the pipe pointing away from the toilet. Love this holder.

Will this hold a double roll or a mega roll?

Yes. It has plenty of space for a large roll
