Im 5"4 and weight 202 pounds my wasit size is 34 my gut hangs over of my pants size by 3 inches so what size do I used ??

If your mid area is curvy, I would go with the medium /large belt. If it only a few extra pounds around your waist you can get away with the small. I hope this helps
