You’ll need a 50 ohm coax with a sma end for the antenna side and whatever connection type you need for the other end you are connecting to. Higher quality coax is best to minimize loss or stay as short as you can on coax length if it’s lower quality coax. Also this antenna has two antenna inside. If you are needing both connections you’ll either need two pieces of coax or a dual/duplex coax to take advantage of that.

Have anyone tried using two of this for 4 x 4 mimo with cat18 modem?

No, I don't use it in that specific way, but I can tell you both ANT outputs don't work at the same time. The second output was designed most likely as backup for the first one.
