Is there a seal between the extension and the original flange? If not, would some sort of seal be necessary?

I used a 100% waterproof silicone sealant between the original and the extension flange. Just to ensure a watertight seal. I also used a thick wax ring when installing the toilet. So far, so good. Hope this helps.

What is the inside diameter opening? Will a 4” pvc pipe fit through the opening? Wanting to use this under a new closet flange install.

I dont know. The toilet is bolted down on top of it.
