How do you apply these to regular nails ? Do i need nail glue or will they stick with a top coat ?

You glue them on, then place the top coat. There are a lot instructional videos on youtube to help too.

They are in size order, the smallest to largest is 5ss, 7ss, 9ss, 12ss, 16ss, 20ss, 30ss. ss is stone size in Swarovski language.

I hope these can be used on tee shirts? I have a hot fix tool and want to use these with that tool, will this be ok?

These are NOT hot fix crystals. You would need to adhere them with a glue, like E6000.
