Do you need a stethoscope to work it?

No you do not need a stethoscope to obtain a systolic reading. You can easily palpate the blood pressure by using the radial pulse and your fingers. This gives you a systolic reading only. To get a diastolic reading, you need a stethoscope.

what size is the cuff?

It's a regular size adult cuff. If you need it to fit a large arm (very overweight or body builder, you may want to get an XL cuff. I don't know if this specific brand is sold in that size on Amazon. The range for this particular cuff is 25.4-40.6 cm. Hope this helps.
