Is the intensity adjustable. And can you pin-point where you want extra work done? Does it have a shiatsu mode?

The intensity has three levels of adjustment. To pin-point an area you can run it in manual mode (which puts it in one position) or in this mode they call BodyMap Pro which works one area. I don't really know what shiatsu is so I can't answer that one. I can tell you that I've used mine everyday for probably 2 years and can't imagine being without it.

How much assembly is required?

The assembly on the Human Touch brand chairs is fairly easy. Most of the work is getting the 110 lb chair in the home and where you want it. BACKSTORE

How is it on the upper back and lower neck? If someone is 6' tall, how high do the upper back rollers come?

I'm 6'1" and it comes up high enough to massage your neck. There are range adjustments you can make on the hand controller to dial it in pretty precisely.
