Do shelves come with this box, if not how do you mount a cable modem that doesn't have mounting capability? The Modem comes from the service provider.

The box does not have shelves, everything is modular. Everything snaps in, mounting brackets, if you use a modem or switch that is not sold by Leviton, no problem there is a module that will allow you to mount 3rd party modem's and switches inside the box. If you install anything that generates heat then it is advisable to get the louvered doors. Go to Leviton.com. I was able to download Visio templates and figure out the exact setup and install before actually doing the job.

will this item fit On-q/legrand devices?

The Legrand cat 5 module that I purchased was terrible and did not fit properly. The one side of the module has ears that are supposed to fit into the pre drilled holes. They don’t fit! I tried and tried and they finally broke off.

Is the panel cover plastic or metal?

I didn't purchase that particular size.... However I have purchased the 14 x 14 with hinge cover and both the box and door are heavy gauge metal

Can this be installed with the knockout for the 120V outlet at the top?

Yes it can. it has two different size knockouts for outlets and two wire run knockouts on one side and 4 knockout holes for wires on the other. It can be flipped either way.
