could I use fabric paint on it?

The cape itself is a very thin material (no lining) and it's polyester. If you're fabric paint is good for polyester, I would say give it a try, and then post a picture and your thoughts as to how it came out, in case someone else wants to try it.

Can I wear it as a winter coat

No, it would not warm enough alone in a cold place! The outer material is comfortable and breathable but not very thick. You could wear it with a lot of layers under it.

Could I possibly iron on a patch to the material?

I believe this is possible, but you would need to use low heat so you don't melt the material. If ironing on the patch requires high heat, I would do it in gradual stages of heat, and be careful to only iron the patch to activate the glue of the patch.

How does the velcro strap work? And is that x strap is it supposed to be seperate, or be done with the velcro?

It's a bit tricky but yes it is velcro. It overlaps across your chest creating the the X look. You take one end of the strap through a ring t sitch it to the tightness you want then with the velcro side facing towards your back you press it to the female side of the strap and there you are.
