Did the costume come with the socks with black stripes?

Yes two black stripes on each sock.

How is this fastened together

It’s 3 pieces , the bottoms have an elastic waist band, the top pulls over like a long sleeve shirt and the head piece pulls over and you stick your face through the hole. You tuck the head piece in to the neck of the shirt

Does the headpiece come with it?

Yes the head piece come with it

Does this fit true to size?

I found it true enough. Little big but I believe that is by design
