Will this work in a rear end and trans of a Ford tractor? Which holds five gallons of hydraulic oil

Don't know but I use these stuff on tuff hydro type pumps like power steering, log splitters, rams but I don't use on brake stuff, jacks and smaller or high tech car trannys. I would try this stuff on big tuff tractor applications-I start low like a half quart for four and a half quarts-see if you like it.

Is it ok to put in a motor grader ?

Don't have one of those but it works wonders in a 1952 John Deere B tractor hydraulics Been using it in there for about 5 years. The leaks have stopped.

can this be combined with mobile 1 15W-30 which is what my exmark vantage takes for hydro oil?

We've not used it in our Exmark. Only in a 1952 John Deere teactor and it has worked wonders with that well used engine.
