I received this item in a medium and it is more like a small. I placed the banana there to show the size. Dogs wont have an issues with the arms. if you own a dog with fatter arms he/she will have issues with walking with this item. the material is great but item needs to be bigger. I would label that size a Extra small or small rather than Medium.
