Can I set voltage to 220 V ?

Not sure, and I'm out of town at the moment. Check spec sheet for it.

If all switches are on, and I plug it into power, will it eventually run itself? Or do I need to press a button each time to produce fog?

Have tonpress the button every time. It can overheat where you have to wait for the green light to come back on to activate it again.

I'm sure it's not the intended use, but does anybody know if you could use this as a thermal pesticide fogger?

I wouldn't recommend it usually those are set to a specific temperature and use, using something not made for that could cause all sorts of issues. This is a regular fog machine and adding anything other than the specific fog fluid is not recommended.
